The internet offers two stories about microplastics: “they’re giving everyone cancer” and “there’s no proof of anything, relax.” Both are lazy. Here’s the actual state of the evidence, sorted into three tiers by how much weight it can bear. We’ll update this page as the science moves — it’s moving fast.
Tier A — Solid human evidence
The chemicals plastics carry disrupt hormones. This is established. Plastics are not inert — they leach their additives: BPA and its replacements, phthalates (the softeners), flame retardants, and in some products PFAS. Decades of human studies link these endocrine-disrupting chemicals to reduced sperm quality, altered fetal development, metabolic disorders, and thyroid disruption. The Endocrine Society — not an activist group, the professional body of hormone physicians — states this plainly. One 2024 estimate puts plastic-chemical-attributable disease costs in the US at ~$250 billion per year.
This matters because every microplastic particle is a delivery vehicle for these chemicals, ferrying them into tissue and releasing them slowly. Even if the particle itself proved harmless (unlikely — see Tier B), the cargo isn’t.
Particles reach and persist in human tissue. Established. Blood, brain, placenta, arteries, lungs, testes — see the full body inventory. Persistence matters: your body has no enzyme that digests polyethylene. What lodges, stays.
Tier B — Strong signals, causation not yet proven
Cardiovascular disease. The NEJM carotid-plaque study is the heavyweight: patients with detectable micro/nanoplastics in their artery plaque had ~4.5× the rate of heart attack, stroke, or death over ~34 months, and their plaque showed more inflammatory markers. One observational study can’t prove the plastic caused the events — but it’s the kind of result that historically precedes a consensus, and follow-up studies are underway now.
Neurological accumulation. Brains carry the highest measured plastic concentrations of any organ tested, the dose rose ~50% between 2016 and 2024, and dementia brains carry more. Correlation, with a plausible reverse-causation explanation — and still the single most unsettling chart in the field.
Fertility. Plastic found in all tested testes; phthalate exposure (Tier A) already linked to sperm parameters; global sperm counts falling for decades. The pieces fit a pattern; the connecting study hasn’t been done.
Tier C — Mechanistic and animal evidence
In lab and animal studies, micro- and nanoplastics reliably produce inflammation, oxidative stress, DNA damage, and cell death, cross the gut barrier, the blood-brain barrier, and the placenta, and in rodents impair memory, alter gut microbiomes, and damage reproductive tissue — typically at doses higher than current human exposure, which is the honest caveat. Animal evidence doesn’t transfer to humans automatically. It’s also exactly the profile you’d expect for a slow-burn population-level harm: the asbestos and leaded-gas playbooks both started here.
What no one has shown
No randomized human trial proves microplastics cause any specific disease — and none ever will, because you can’t ethically feed people plastic for a decade. The evidence will stay observational and mechanistic, the way it did for tobacco and asbestos. You will have to make your call before the final verdict. Everyone alive right now does.
Our call
The presence data is fact. The chemical-harm data is fact. The disease links are probable-and-emerging. Exposure is rising ~50% per decade, and the high-impact reductions cost almost nothing.
